In agreeing to participate as a client of Cast Performance, I affirm that my general health allows that I may participate in any exercise and/or training programs under the arrangements of my coach.

Furthermore, I am aware of the limitation(s) that my general health may place on performing exercises of a vigorous nature and of my responsibility for seeking medical advice in that regard. Also, in the future, I will immediately notify my coach and/or Cast Performance should I become aware of such limitations or have changes with regard to my medical condition(s) or my medication(s).

I am aware of the possibility of accidental or other physical injuries during the exercise and/or training programs under the arrangements of Cast Performance and/or my coach.

In consideration of participating with Cast Performance, I do hereby agree to assume all risks of such injury and will hold Cast Performance harmless from any and all liability, actions, causes, claims, and demands of any or every kind and nature whatsoever, which I now have or which may arise out of or in connection with my participating in activities arranged by Cast Performance and my coach. The terms thereof shall serve as a release and assumption of risk for me and my heirs, personal representatives, and administrators, as well for all members of my family, including my minors.

I have read this agreement and understand the exercises and/or training programs in which I will be engaged. I have also informed my coach of any physical limitation(s) that could possibly restrict my participation in any physical activity.
